Overview
A modern facelift is not skin tightening. Pulling skin produces the swept, operated look everyone recognises and fears; it also relapses quickly because skin is not a load-bearing tissue. Contemporary technique works on the SMAS layer beneath — repositioning it so the skin is redraped without tension.
Face and neck are assessed together. In most patients over forty-five, the neck is the giveaway, and a facelift that ignores it delivers a result that reads as incomplete from every angle except straight on.
