What is Double Incision Top Surgery?
Double incision top surgery, also called double incision mastectomy or bilateral mastectomy, is a gender-affirming procedure primarily used for transgender men and non-binary individuals with larger amounts of chest tissue.
The term "double incision" refers to the technique, where two horizontal incisions are made along the lower border of the pectoral muscles. Through these incisions, the surgeon removes the underlying breast tissue and excess skin to create a flatter, more masculine chest contour.
In most cases, the nipples and areolas are removed, resized, and grafted back onto the chest in a process known as free nipple grafting. This step ensures the nipples are repositioned to complement the new chest shape and achieve a natural-looking masculine appearance.
Some patients may require additional contouring to prevent "dog ears," a term for excess skin or fat that can form at the edges of the incisions, resulting in an uneven chest shape. Your surgeon will discuss this with you during the consultation.
