Breast Implant Incisions and Positioning

There are three primary types of incisions used in breast implant surgery, with the most common being the inframammary incision, which is made along the lower crease of the breast. It's essential to understand that all surgical incisions will leave a scar, but with proper post-operative care, these scars will gradually fade.

By around three months after surgery, most scars are minimally noticeable, especially when wearing clothing or when the arms are relaxed by your side. Your surgeon will provide detailed information on how each incision option might look over time and will recommend the most suitable choice based on your needs. In most cases, the inframammary approach is regarded as the gold standard technique for breast implant placement, as it offers the best access and results in well-concealed scarring.

Types of Breast Implant Fill Material: Silicone vs. Saline

All modern breast implants feature a silicone outer shell, regardless of the internal fill material. The two primary types of breast implant fill materials are saline (saltwater) and silicone gel.

Saline-Filled Breast Implants

Saline implants are often described as having a texture similar to a water bed, which can sometimes result in a rippling effect. This rippling is a known issue, particularly for women with minimal breast tissue, as the ripples may become visible through the skin. The use of round, smooth-surfaced implants can help reduce this risk. Overfilling saline implants makes them feel firmer but can result in a more spherical, less natural shape. If a saline implant were to deflate, it typically happens within 2 to 3 hours after surgery, and the body absorbs the saline solution without any adverse effects. However, saline implants have a higher risk of capsular contracture and "bottoming out" compared to silicone implants.

Silicone Gel-Filled Breast Implants

Silicone gel implants have a much lower risk of rippling due to the consistency of the silicone gel, which mimics natural breast tissue more closely. They maintain their shape better and offer a more natural look and feel, even during movement. Silicone implants have a low risk of deflation, but in rare cases, they can rupture, especially due to traumatic chest injury. Modern silicone implants are made with medical-grade silicone that doesn't migrate if a rupture occurs, but a rupture might increase the risk of capsular contracture. If a silicone implant ruptures, surgical removal and replacement are necessary.

Saline vs. Silicone: Which Is Best?

While saline implants were popular about 20 years ago, advancements in silicone gel technology have made silicone implants the preferred choice today. Silicone implants offer numerous advantages, including:

  • A more natural look and feel
  • Better retention of shape
  • High-quality cohesive gel that minimizes the risk of leakage
  • Reduced risk of sudden deflation
  • Easier for surgeons to predict and control projection
